Information sheet for families


Collecting personal data for the dyslexia research database has been approved by the School of Psychology Research Ethics committee (reference PSC-432 date 25/09/2018)

Purpose of the database

We are starting a database of families who have a school age child who has dyslexia.  We are inviting you to sign up to this database, providing us with your contact details so that we can contact you about future research studies.  Consenting to provide your contact details for our database does not mean that you are consenting to take part in a research study.  Any future research studies would be fully explained to you by the researchers organizing them and your consent to take part would be gained.

What details am I being asked to provide?

We are asking you to provide:

Your name, your child’s name, your child’s date of birth, your phone number, your email and your address, to indicate whether your child has a diagnosis of dyslexia or is in the process of being diagnosed.

How will my details be stored?

Your details will be stored in hard copy on the consent form.  This consent form will be stored securely in a locked filing cabinet.  Your details will also be transferred to an excel file, which will be password protected and stored on the University’s central server. Your details will be stored for a period of 5 years, after which they will be securely and safely destroyed. If you wish to update your data you can contact me using my details below.

Who will have access to my details?

I (Dr Hannah Nash) will manage access to your details.  With your permission I will share you contact details with other researchers working at the University of Leeds so that they can contact you about research studies you may be interested in.  Otherwise your details will remain confidential and be stored securely.

Right to withdraw your details

If you would like to withdraw your details before the 5 year period is complete please contact me (Dr Hannah Nash) to request this.
